(Mobile Integrated Healthcare and Community Paramedic) programs are a new innovative way to deliver community-based care that aligns with value base medicine initiatives in support of population management, patient-centered care, healthcare accessibility, reduced costs, and high quality healthcare. Our approach is designed to augment other patient-centered delivery models by providing an optimized mix of healthcare and patient navigation while utilizing community paramedic interventions and additional service lines such as telephonic triage, alternate (non-ED) destinations, in-home assessments, and the use of specialty providers as part of the EMS system response (Doctors, NP’s, PA’s, RNs, social workers, mental health counselors, and behavioral health specialist) Our programs provide a data-driven and proven solution to assist healthcare organizations and institutions with filling gaps in local healthcare delivery systems and community needs. The programs utilize EMS providers with an expanded scope of practice, using non-emergent resources, responding into the community to care for patients.
